Community Health Initiative’s Referral Program
Dignity Health’s Community Health Initiative Program accepts referrals from community agencies and organizations to assist individuals and families with health insurance enrollment, education, navigation among other things. Here are some of the services we are able to offer:
- Provide assistance and facilitate the process of submitting an application to the Department of Human Services.
- Prescreen to determine what program client is eligible. If all avenues are exhausted, we try to connect them with other community resources.
- Assist with health insurance education and support with using health plan benefits.

In 2024, the CHI program received over 200 referrals, 151 households received services and more than 130 individuals completed enrollment into a health insurance program. Thank you to the following agencies for the referrals:
