Our Process

The Community Health Initiative is committed to assist individuals with the application process until there is a determination outcome. The following information explains in detail our process.

NOTE: For Cal-Fresh applications, staff is only able to assist clients up to the application & document submission. The outcome determination will be the responsibility of the applicant. If the individual/family receives CalWORKs assistance, they should contact their case worker, as CHI staff will not be able to assist with their needs.

 

PRESCREEN


Staff will evaluate your household to determine which program(s) you are potentially eligible for. They will ask for general contact information, identity, and income for all your household members. They will do a pre-evaluation and let you know which program(s) you/your household members are potentially eligible for. If you are still interested, we will assist you with the next steps to submit an application.

 

GATHER DOCUMENTS


Based on your pre-evaluation, the team will be able to determine which documents are needed to submit your application; this may also include signed consent forms which will allow program staff to assist you. Our program’s best practice is to submit applications with needed documents as it will support us when we are advocating on your case. You have at least one week to submit needed documents to our program. NOTE: We do not submit applications if we do not receive the documents.

 

APPLICATION COMPLETION


The Certified Enrollment Counselor assigned to your case will contact you to complete the application. We can do this via a phone or an in-person appointment.

APPLICATION & DOCUMENT SUBMISSION


The Certified Enrollment Counselor (CEC) completes the application via the corresponding portal, completes forms and gathers documents. The complete application is now submitted to the corresponding program. The CEC will provide you copies of the application submitted along with copies of the documents provided.

EDUCATION


The Certified Enrollment Counselor will share information regarding next steps for your application, and will provide you with educational material if this is your first time receiving our assistance.

OUTCOME DETERMINATION


Program staff will keep communication to provide you with updates we may have regarding your case. This may include the health insurance program needing documents or an eligibility worker needing to schedule an interview with you. We will contact you as necessary until a determination on your case has been made and inform you of the case outcome. 

 

UTILIZATION SERVICES


Program staff will call you to follow-up and ensure you have used your health insurance and received your health insurance cards. Staff will be able to assist you with scheduling appointments, selecting and/or changing doctors or health plans. Now that you are covered, it is time to take advantage of your FREE preventive services!
